INSURANCE PRIMER · ASK THE PLAN

Coverage follows the product, the reason and the plan.

An insurance plan may pay for the same ingredient when it is prescribed for one reason but not another.

Three connected points labelled PRODUCT, REASON and PLAN. PRODUCT REASON PLAN

Coverable is not guaranteed

As of September 2026, Medicare can cover a weight-loss medicine when it is prescribed for another covered use, such as diabetes or heart risk. Each plan can still use its own medicine list and require approval before it pays.

Ask whether the exact product is on that list and what information the plan needs before it will pay.

Ask in a useful order

  • Does the plan treat this visit as “in network,” meaning it uses a clinician in the plan?
  • Does the lab need a particular location?
  • Is the exact product on the plan’s medicine list?
  • Does the plan need to approve it before paying?
  • What do I pay before my plan starts sharing the cost?
  • Does a cash payment count toward that amount?

Keep the reference details

Write down the date of the call, the answer, and any reference number the plan gives. If the answer changes, those details make the next call easier.

A coverage answer is not a medical recommendation. It only explains what the plan may pay.

CHAPTER RECAP

Keep this: use the exact product name, the reason for use and the plan’s own words. “Covered” without those details is too loose.
